how can I get a birth certificate as a minor?



Answer:

In Arizona, the Department of Vital Records, housed in the Arizona Department of Health Services is the agency where a person would apply for a certified copy of their birth certificate.

Arizona Administrative Code, R9-19-403, states that a certified copy of a birth certificate "shall not be issued to an unemancipated registrant (person asking for a copy of their own birth certificate), under 18 years of age without the permission of at least one parent."
