Where can I get a form to take to superior court for a legal name change?

Answer:

You are absolutely right about filing for a name change in superior court. A.R.S. § 12-601 says that adults can file for a name change in the county of his or her residence. The parent or guardian of a minor may file an application for the name change of a minor. The court will determine the best interests of the child in determining if the name change should be granted. Forms are available on the Arizona Judicial Department website which can be linked through our School Stuff page. Some stores also have legal forms for purchase.
